Case Manager

 

Division:  Case Management Program - Central Aroostook County

Northern Maine General has one opening for a full-time Case Manager.  Case Managers work with adults with intellectual disabilities and collaborate with parents, community support workers and other health professionals.  The desired candidate would provide direct treatment services on an as-needed basis and help clients succeed through a person-centered plan.  A bachelor’s degree with a concentration in social work or related field is necessary.  Candidate must have a valid driver’s license and be able to travel throughout Aroostook County if necessary.

Plan Coordinator

 

Division:  Home & Community Supports Division

Type:  Full-time

 

Northern Maine General is seeking a Plan Coordinator (PC) who is responsible for developing residential treatment plans in collaboration with an individual’s team, and coordinating, providing oversight, and consultation to shared living homes throughout northern and central Aroostook County.  Dependent on caseload, the PC may also supervise waiver home managers, providing quality assurance measures to assure the managers are operating in compliance with all regulatory requirements. 

 

The PC must be able to multitask and work both independently and collaboratively in a team-oriented environment and in the community.  The PC must be able to understand and maintain professional boundaries and ethical standards, respond calmly and professionally to crisis situations, effectively present information, and respond to clients, guardians and families, staff, and outside agencies.

 

Candidate must have a bachelor's degree in a field of study applicable to the human services field or have a minimum of 3 years’ experience in the provision and coordination of services for individuals with intellectual and developmental services.  Proficiency in Microsoft Office is a must.  It is preferred that the candidate have training in psycho/social rehabilitation, interviewing and counseling, community mental health, mental health and aging, and group processes.

Mental Health Specialist

 

Division: Franciscan Home, Eagle Lake

Type: Full-time and part-time

 

Northern Maine General is seeking a dedicated individual to serve as a Mental Health Specialist at our Franciscan Home in Eagle Lake.  Mental Health Specialists work in a community residential program, caring for individuals who are diagnosed with chronic mental illness.  The desired candidate would promote an individual’s development of interdependence, self-directed care, and individually created support systems and community involvement.  Candidate must be at least age 18, have a high school diploma or equivalent, and have a valid driver’s license.

Contracted Opportunities

 

Shared Living Providers Needed from Fort Kent to Portage

 

Northern Maine General (NMG) is currently seeking individuals or families interested in supporting an adult with intellectual disabilities or autistic disorders.  There is a need for Shared Living Providers (SLP) from Fort Kent to Portage.  The primary provider must be at least 21 years of age. 

 

There are two models of shared living: 

  • The adult seeking services moves in with the SLP.
  • The SLP moves in with the adult seeking services.   

 

Shared Living allows an adult with disabilities to live in a family-style setting and become a member of the household, the family, and their community. The SLP might help the person with their medical appointments, administer their medications, help them complete their daily living skills, and support them to be involved in their community. 

 

The SLP must enter a contracted agreement with an oversight agency to provide services.  They must become a Certified Direct Support Professional and comply with the Shared Living Handbook provided by the Department of Health and Human Services. The contract includes a monetary stipend for the service provided.
